Please find below the financial forms for work done on behalf of the PTA. These forms can be downloaded, filled out electronically, and then returned to treasurer@beverlyclearyschool.org for processing.

• PTA Cash Box Form
• PTA Cash Counting Form
• PTA Expense Reimbursement Form
  Use this form to request reimbursement for a budgeted PTA expense.
  
• PTA Check Request Form
  Use this form to request payment for a third-party vendor for a budgeted PTA expense.
  
• PTA Funding Request Form
  Use this form to request funding for an event/initiative that is not already in the PTA budget.
